Với những thành công ở các phiên bản Romance of Three Kingdoms khác. Nhà phát triển game Koei không muốn chỉ dừng lại ở đó, mà muốn làm vang dội danh tiếng của mình với dòng game Romance of the Three Kingdoms của mình nên Koei quyết định xây dựng, phát triển và cho ra đời Romance of Three Kingdoms IX để tiếp nối cho series RTK thành công của mình. 

Nội dung chính của game xoay quanh cuộc chiến giữa các nước chư hầu vào thời loạn lạc với ba nước mạnh nhất và được đề cập tới nhiều nhất là: Ngụy - Thục - Ngô vào thời Tam Quốc Chí. Khác với các phiên bản tiền nhiệm trước đó, ở phần 9 này người chơi sẽ luôn vào vai các vị tướng lãnh đạo. Nhiệm vụ của người chơi là đi chinh phục từng thành phố hay thị trấn trên mỗi bản đồ về dưới trướng của mình, để rồi từ đó thuê dân để làm kinh tế, xây dựng quân đội và đi đánh chiếm quân địch.

Romance of the Three Kingdoms IX bao gồm 15 cốt truyện dựa trên sự kiện lịch sử, 5 cốt truyện thử thách người chơi và 5 cốt truyện do người chơi thay đổi dựa trên quá trình chơi và cũng sẽ những cái kết khác nhau tương ứng với thay đổi đó.
