Dựa vào các đánh giá từ phần đầu của game mà Raven Shield định hướng cho phần 2 này theo hướng "thực tế" như các tựa game bắn súng chiến thuật khá như Tom Clancy's Rainbow Six, lấy đó làm nguồn cảm hứng để phát triển. Soldier of Fortune II được phát triển dựa trên id Tech 3 nên đồ họa của phần này nhìn chân thực hơn so với các tựa game FPS cùng thời. Và một lần nữa cũng như phần đầu tiên, Double Helix tiếp tục đẩy giới hạn của mức độ bạo lực và máu me của tựa game lên một tầm cao hơn nhờ engine id Tech 3 này.

Cốt truyện của game thay vì lấy chủ đề vũ khí hạt nhân như phần 1 mà ở phần này game lấy chủ đề về vũ khí sinh học. Câu chuyện bắt đầu khi tay lính đánh thuê Mullins cùng người đồng đội Madeline Taylor của anh đi tới Colombia để tìm hiểu về nguyên nhân bùng phát dịch ở một thị trấn nhỏ. Và sau này anh mới biết được rằng cơn dịch bùng phát là do virus có tên Romulus gây ra, và tổ chức sở hữu nó có tên là Prometheus, một tổ chức mà Mullins truy lùng từ lâu. Và dựa vào Romulus mà Prometheus đã gửi thư đe dọa đến Hội nghị thượng đỉnh G8. Xuyên suốt tựa game ta khám phá ra có gián điệp bên trong Tổ chức lính đánh thuê mà Mullins đang hoạt động đã tuồn thông tin cho đám khủng bố. Liệu Mullins cùng với những người đồng đội của mình sẽ làm gì trước mối nguy thảm họa sinh học và anh sẽ triệt phá tổ chức Prometheus ra sao?!
